Septal Ulcer

Septal ulcer is ulceration of the nasal septal mucosa, due to trauma (nose picking), chemical irritants (cocaine), infection (TB/syphilis), granulomatosis (Wegener/sarcoid), or radiation. Clinical: crusting, epistaxis, nasal obstruction, pain. Treatment: identify/treat cause; avoid trauma/irritants/cocaine; saline/moisturizing; antibiotic ointment; treat underlying infection/granulomatous disease; surgical repair if perforation/saddle-nose deformity.

Crusting/epistaxis; nasal obstruction/pain; by traumatic/chemical/infectious/granulomatous

Causes and Pathogenesis

Trauma/irritant/infection/granulomatous causes septal mucosal ulceration and possible perforation.

Treatment

Treat cause; avoid trauma/cocaine; saline/moisturizing; antibiotic ointment; repair if perforation

Preventive Measures

Avoid nose picking/cocaine; humidification; treat underlying disease.
