Choledochal Cyst

Choledochal cyst is congenital cystic dilation of biliary tree (Todani classification), presenting in children/adults with jaundice, RUQ mass, cholangitis, or pancreatitis; malignant risk (cholangiocarcinoma). Treatment: complete surgical excision of cyst; bilioenteric reconstruction (Roux-en-Y hepaticojejunostomy); treat acute cholangitis pre-op (antibiotics/drainage); monitor for anastomotic stricture/cholangitis/malignancy; long-term follow-up imaging.

Jaundice; RUQ mass; cholangitis; pancreatitis; congenital dilation

Causes and Pathogenesis

Abnormal pancreatobiliary junction causes cystic dilation and stasis.

Treatment

Complete excision; bilioenteric reconstruction; monitor malignancy
