Hypokalemia is serum potassium below 3.5 mmol/L. Causes: loop/thiazide diuretics, GI losses (vomiting/diarrhea), hyperaldosteronism, Cushing, renal tubular acidosis, and insulin/glucose therapy. Clinical: muscle weakness/cramps, fatigue, constipation/ileus, rhabdomyolysis (severe), and cardiac arrhythmias (U waves, QT prolongation, torsades). Treatment: oral potassium chloride for mild-moderate; IV potassium (carefully, via controlled infusion) for severe/symptomatic; treat underlying cause; check magnesium.
Muscle weakness/cramps; fatigue; constipation/ileus; arrhythmia; U waves on ECG; rhabdomyolysis (severe)
Renal/GI potassium loss or transcellular shift lowers serum K, impairing neuromuscular and cardiac membrane repolarization.
Oral potassium chloride; controlled IV potassium for severe; replete magnesium; treat diuretic/hyperaldosteronism cause; monitor ECG/K
Monitor K on diuretics; replace GI losses; treat hyperaldosteronism.
It is recommended to visit the Internal Medicine department; an appointment can be booked on this platform before the visit.
