Exfoliative dermatitis (erythroderma) is generalized erythema/scaling involving >90% body surface, from psoriasis/atopic dermatitis/drug reaction/seborrheic dermatitis/cutaneous T-cell lymphoma. Treatment: emollients/wet wraps; mild-moderate topical corticosteroids; systemic steroids for severe; treat underlying cause (biologics/MTX for psoriasis; stop drug; immunosuppression for CTCL); fluid/electrolyte/protein support; temperature regulation; monitor for infection/sepsis/high-output cardiac failure; hospitalize if severe; follow-up.
Generalized erythema/scaling; >90% BSA; by psoriasis/drug/atopic/CTCL
Underlying dermatosis/drug/lymphoma causes widespread inflammation and scaling.
Emollients; topical/systemic steroids; treat cause; fluid/protein support
Early treatment of underlying dermatosis; avoid trigger drugs.
