Impetigo herpetiformis is severe pustular psoriasis occurring in pregnancy (third trimester), presenting with erythematous plaques with peripheral pustules, fever, and hypocalcemia. Treatment: hospitalize; systemic corticosteroids; cyclosporine; acitretin (avoid if breast-feeding); supportive care (fluid/electrolyte/calcium); monitor mother/fetus; deliver if severe/fetal distress; treat post-partum relapse; follow-up dermatology/obstetrics; document as pustular psoriasis; avoid trigger drugs.
