Heat Urticaria

Heat urticaria (cholinergic) is small pruritic wheals on heat/exercise/emotional stress, from cholinergic mast-cell activation, presenting with punctate wheals on trunk/neck after hot shower/exercise. Treatment: H1 antihistamines; avoid overheating/hot showers; gradual exercise desensitization; cool environment; oral antihistamines (cetirizine/fexofenadine); omalizumab for refractory; rule out anaphylaxis; follow-up allergy/dermatology.

Causes and Pathogenesis

Cholinergic activation of mast cells causes small wheals.

Treatment
