Erythema Nodosum

Erythema nodosum is septal panniculitis presenting with tender erythematous subcutaneous nodules on extensor shins, often from streptococcal infection/sarcoidosis/IBD/sulfonamides/sarcoidosis. Treatment: bed rest; leg elevation; compression; NSAIDs; potassium iodide for persistent; treat underlying cause (antibiotics for strep; treat sarcoidosis/IBD; stop culprit drug); systemic corticosteroids for severe; follow-up dermatology/internal medicine; biopsy if atypical; ESR/CRP/CXR/strep screen.

Causes and Pathogenesis

Septal panniculitis from immune reaction to infection/drug/systemic disease.

Treatment

NSAIDs; rest/elevation; treat cause; KI; steroids if severe

Treat strep; avoid trigger drugs; manage IBD/sarcoidosis.
