Nodular vasculitis (erythema induratum of Bazin) is recurrent tender subcutaneous nodules on posterior lower legs (calves), often ulcerating, associated with TB (M. tuberculosis), in women. Treatment: investigate TB (PPD/IGRA/chest X-ray); oral anti-TB therapy (isoniazid/rifampin/pyrazinamide/ethambutol) for 6-9 months; rest; leg elevation; compression stockings; topical/intralesional corticosteroids; treat secondary infection; follow-up dermatology/infectious disease; monitor recurrence; distinguish from erythema nodosum/pancreatic panniculitis.
Tender subcutaneous nodules; posterior lower legs; ulcerating; women; TB association
Hypersensitivity to TB (or other antigens) causes lobular panniculitis/vasculitis.
Anti-TB therapy; rest/elevation; compression; topical/intralesional steroids
TB screening; early treatment; avoid cold/trauma.
It is recommended to visit the Dermatology/Infectious-disease department; an appointment can be booked on this platform before the visit.
