黑痣
Dysplastic nevus (dysplastic nevus syndrome, B-K syndrome) is often misdiagnosed as superficial spreading melanoma; carries melanoma risk. Familial tendency. Melanocytic hyperplasia with atypia not reaching melanoma in situ. Defect may have genetic basis; lesions anywhere on body, up to 6 cm, irregular shape/borders/color (gray/dark brown/pink); central nodule in a macule warrants melanoma suspicion. Excision is first-line.
Black nevus
Appears in childhood; 1/5-1/3 histologically resemble melanoma. Atypia/proliferation correlate with sun exposure. Linked to chr11 deletion and testicular germ cell tumors; familial melanoma/dysplastic nevus accompany endocrine polyglandular/autoimmune polyglandular syndromes. Genetic basis: autosomal dominant; abnormal gene at 1p35 or 9p21; p16/CDKN2A mutation causes abnormal cell differentiation. UV-induced photoproducts cause malignant transformation.
