Rare; may arise in preexisting blue nevus/Ota nevus or de novo. Superficial, may ulcerate; local nodal or widespread systemic metastasis. Common necrosis, no junctional activity.
Nodule
Arises from abnormal dermal melanocyte aggregation; associated with pigmented nevi, cardiac myxoma, mucocutaneous myxoma, nodular mastocytosis. Blue-gray appearance is optical (long wavelengths absorbed, short blue reflected). Explosive blue nevus relates to sun exposure. Mechanism: melanocytes in mid-dermis, may extend to upper dermis/subcutis; elongated spindle cells scattered among collagen; melanophages; ocular lesions may involve periosteum.
Surgery; supportive care
No effective prevention; early detection/diagnosis. Avoid friction/injury to facial nevi; itching/burning/growth/darkening/ulceration/bleeding warrants concern.
