Hemolytic streptococcal gangrene (necrotizing fasciitis) is rapidly progressive deep soft tissue necrosis by Group A Strep; pain out of proportion; sepsis; emergent surgery.

Symptoms

Pain out of proportion; rapid swelling; bullae; skin discoloration; sepsis; fever; after minor trauma

Causes

Group A Streptococcus; deep fascia; surgical emergency.

Treatment

Emergent surgical debridement; clindamycin + penicillin; ICU; IVIG for toxic shock
