Paranoid mental disorder (persistent delusional disorder) is characterized by non-bizarre, organized delusions (persecutory, jealous, grandiose, somatic, or eroto-manic) lasting at least 3 months, without prominent hallucinations or formal thought disorder. Function is not markedly impaired apart from the delusion. Onset is later than schizophrenia; risk factors include sensory impairment, immigration, and substance use. Treatment: antipsychotics and psychotherapy; engagement is often difficult.
