Bowen's disease

肿瘤外科

Common symptoms

角膜烧伤、角膜上皮糜烂、角膜有菌苔或溃疡

The pathology of disease

This condition was long mistaken for a precancerous lesion, but it is now recognized as true intraepidermal squamous cell carcinoma — also called in situ squamous cell carcinoma, cutaneous carcinoma in situ, or intraepidermal SCC.

Common Symptoms

Corneal burn; corneal epithelial erosion; corneal plaque or ulcer

Causes

The exact cause and mechanism are unknown. Most are primary, possibly related to: (1) arsenic exposure — some patients have a history, with high arsenic in lesions; (2) viral — may arise on epidermodysplasia verruciformis (HPV5), relation under study; (3) external irritation — some lesions follow trauma or insect bites; (4) nevus syndrome — many lesions arise in pre-existing moles; (5) sunlight — lesions favor sun-exposed sites; (6) genetics — familial cases reported. The earliest lesions are thought to occur at the follicular apex and infundibulum, especially the outer root sheath at the sebaceous level. Carcinogenesis of conjunctival/epithelial cells may relate to UV irradiation. UV damages DNA; when repair is delayed or fails, normal epithelial cells may transform into tumor cells.

Treatment

Drug therapy; supportive care

Prevention

Bowen's disease daily prevention: protect the eyes during work.
