Merkel cell carcinoma, also called cutaneous trabecular carcinoma, primary cutaneous neuroendocrine carcinoma, primary small cell carcinoma of skin, or cutaneous APUDoma, was first reported by Toker in 1972. About 80% are associated with Merkel cell polyomavirus; the other 20% suggest other factors.

Common Symptoms

Nodule

Causes

Etiology unknown; possibly related to environment, genetics, diet, pregnancy emotion/nutrition. Pathologically: uniform round/oval cells in sheets, in dermis, often infiltrating subcutis/muscle; cells 2-3x mature lymphocytes, oval nuclei, fine dispersed chromatin, prominent nuclear membrane.

Treatment

Surgery; drug therapy; supportive care

Prevention

Keep the area clean; wash vulva daily; change underwear; avoid chronic discharge irritation. Nodules/ulcers/papillary masses or white lesions on vulva should be checked. Middle-aged/elderly women should have regular gynecologic screening; treat vulvar leukoplakia/papilloma early.
