Hidradenoma papilliferum is a vulvar/perianal tumor of adult women; uncommon, mainly age 25-40; vulvar incidence is 4x perianal.
Granuloma; nodule
Caused by sweat gland epithelial hyperplasia; exact cause unknown. Possible factors: chronic friction/irritation, spicy/fried diet; genetics; radiation/irritants/inflammation. Histology: dermal, well-circumscribed, fibrous capsule, usually disconnected from overlying epidermis; ducts/cystic spaces with papillary folds projecting into lumens; lined by single row of columnar cells with oval pale nuclei showing decapitation secretion; surrounded by small cuboidal myoepithelial cells with dark basophilic nuclei. Luminal cells are PAS-positive, diastase-resistant, positive for nonspecific esterase/acid phosphatase, negative for phosphorylase/eccrine enzymes; peripheral myoepithelial cells alkaline phosphatase-positive. EM shows membrane-bound secretory granules and lipid droplets at apex with decapitation secretion.
