丘疹、结节、乏力
Sebaceous adenoma is a benign tumor of nests of sebaceous cells, mostly forming small cystic spaces; stroma is fibrous connective tissue. Some show histiocytes/foreign-body giant cells without cholesterol crystals. Ductal dilatation or eosinophilic metaplasia may be present. Mucicarmine stains focal positive in ductal epithelium/luminal content; sebaceous cells negative.
Papule; nodule; fatigue
Benign tumor from incompletely differentiated sebaceous glands. Well-circumscribed, multilobulated, pseudocapsule, mid-lower dermis. Lobules contain two cell types: undifferentiated germinative cells (basal cells) at periphery, and mature sebaceous cells centrally, with transitional forms. Some lobules are mostly basaloid, others mostly mature sebaceous; most are roughly equal with germinative cells surrounding sebaceous nests. Fat stain shows lipid in sebaceous/transitional cells. Larger central cysts form from disintegrated sebaceous cells; squamous keratinizing foci represent ductal differentiation.
Surgery; drug therapy; supportive care
