Adenolymphoma

肿瘤外科

Common symptoms

驻站医、生长缓慢、腮腺无痛性肿块、淋巴结退化

The pathology of disease

Adenolymphoma, also called papillary cystadenoma lymphomatosum or Warthin tumor, was first reported by Albrecht and Arzt (1910); Warthin described 2 cases in 1929. Arises from ectopic salivary tissue within adjacent lymph nodes. Lymphoid component is mainly T cells with few B cells; represents cell-mediated immune response, not neoplastic.

Common Symptoms

Slow growth; painless parotid mass; lymph node degeneration

Causes

Gross: usually small (3-4 cm), smooth, slightly lobulated, thin complete capsule, round/oval, soft, sometimes cystic; cut mostly solid, gray-white cheesy uniform; cystic areas contain brown clear mucinous/colloid/milk-like fluid. Microscopy: epithelial and lymphoid tissue; irregular large ducts/cysts with papillae projecting into lumens; eosinophilic amorphous material, cholesterol clefts, inflammatory cells; double-layered epithelium (inner tall columnar granular eosinophilic; outer cuboidal/polygonal round vesicular pale nuclei); no atypia; squamous metaplasia, occasional mucous/sebaceous/ciliated cells; lymphoid stroma with follicles and germinal centers. Biology: slow-growing, rarely >10 cm; local recurrence 5.5-12.2% but often multifocal/bilateral; malignant transformation rare.

Treatment

Drug therapy; surgery; supportive care

Prevention

Break bad habits; healthy lifestyle; eat anticancer foods (broccoli, cabbage, citrus, garlic, apricot, fig, sweet potato, bitter melon).
