The pathology of disease

Carotid body tumor is a rare chemoreceptor tumor, also called paraganglioma. The carotid body lies at the carotid bifurcation, encapsulated, ~3.5 mm, vascular/neural, supplied by carotid branches, innervated by cervical sympathetic/glossopharyngeal/vagus/hypoglossal nerves. Gross: red-brown, round/oval, lobulated, encapsulated; cells polygonal with eosinophilic vacuolated cytoplasm. Age 30-40; malignant rate 5-10%.

Common Symptoms

Tremor; progressive neck mass; syncope; hoarseness; dyspnea; neck pulsation; systolic murmur; dysphagia; dizziness; neck enlargement

Causes

A chemosensory tissue tumor in the carotid bifurcation adventitia; nonchromaffin paraganglioma. Related to chronic hypoxia (higher incidence at high altitude). Long-term hypoxic stimulation causes compensatory hyperplasia; familial cases are often bilateral.

Treatment

Surgery; drug therapy

Prevention

No effective prevention; early detection/treatment.
