The pathology of disease

Germinoma arises from primordial germ cells; favors pineal region, then suprasellar cistern. Male adolescents (pineal); females (suprasellar). Highly radiosensitive. Unencapsulated, no calcification/hemorrhage/necrosis/cyst; low-grade, infiltrative, seeds subarachnoid/ventricular system. Two cell types: epithelial and lymphoid. Rare familial; reported in Klinefelter (47,XXY). Chromosome anomalies including i(12p).

Common Symptoms

Visual disturbance; diabetes insipidus; headache with vomiting; somnolence

Causes

Arises from primordial germ cells that failed to migrate normally during week 4 of embryogenesis. Chromosomal abnormalities (nonrandom aneuploidy/translocations; i(12p) in 80% testicular GCT). Primordial germ cells migrate from yolk sac wall through dorsal mesentery to genital ridge; arrested migration may become neoplastic.
