Testicular lymphoma is rare; ~5% of testicular tumors (Doll: 360/7743 = 4.6%). It is the most common testicular tumor in patients over 60.
Weight loss; nodule; anorexia
(1) Etiology: some have prior trauma/orchitis/spermatic-cord filariasis; cryptorchidism rare. Exact cause unknown. (2) Mechanism: size varies; often involves epididymis/spermatic cord/seminal vesicle; tunica/scrotal skin rarely. Histology: most intermediate/high-grade; diffuse large B-cell most common. Inexperienced pathologists may misdiagnose as seminoma; consider lymphoma in elderly. Prognosis: nodal involvement/stage II (bulky nodes) = poor; size >9cm, B symptoms, age >65 = poor. Gross: testis enlarged, tunica intact; epididymis/seminal vesicle often involved; uniform/nodular, hemorrhage/necrosis, gray/yellow/red/brown. Microscopy: almost all NHL; Hodgkin extremely rare. Almost all diffuse (historically 'reticulum-cell sarcoma' → lymphocytic); Rappaport: diffuse histiocytic 76%, poorly differentiated lymphocytic 14%, mixed 3%, other 7%; IWF: intermediate 47, high 21, low 1 of 69.
