Benign Duodenal Tumors

肿瘤内科

Common symptoms

腹部肿块、驻站医、腹部不适、呕吐、黑便、食欲减退、腹痛、恶心、嗳气

The pathology of disease

Benign duodenal tumors are rarer than malignant (ratio 1:2.6-6.8). Though benign, some have high malignant potential; borderline lesions hard to diagnose microscopically. Position near biliary/pancreatic drainage, fixed location, narrow lumen → various symptoms/severe life-threatening complications. Surgical management challenging.

Common Symptoms

Abdominal mass; abdominal discomfort; vomiting; melena; anorexia; abdominal pain; nausea; belching

Causes and Pathogenesis

(1) Etiology: unknown. (2) Mechanism: common pathologic types: (a) Adenoma: papillary/polypoid, solitary/multiple, most common. Types: tubular adenoma (solitary pedunculated, bleeding, broad-based = larger; true neoplasm); papillary/villous adenoma (21-47% of duodenal cancers arise from villous adenoma; broad-based, friable, bleeding, malignant rate 28-50%); Brunner tumor (submucosal polypoid hamartoma; rare malignant change); non-neoplastic polyps (hyperplastic/inflammatory; self-resolving); GI polyposis syndromes (Gardner/Peutz-Jeghers/FAP; multiple throughout GI; duodenal lesions may malignant-transform). (b) Leiomyoma: mesenchymal origin; discrete smooth-muscle bundles; solitary; round/oval/lobulated; <1cm to >10-20cm; intraluminal/intramural/extraluminal; firm; mucosal surface vascular → bleeding; malignant rate 15-20% if mitoses active; most in descending/horizontal duodenum; average 56.6y. (c) Carcinoid/neuroendocrine tumors: from ECL/APUD cells; mostly asymptomatic (except gastrinoma); 1-5cm; 60% benign; gastrinoma/somatostatinoma/gangliocytic paraganglioma; proximal duodenum/periampullary; 70% of gastrinomas in Zollinger-Ellison are duodenal. (d) Other: lipoma/hemangioma/fibroma/hamartoma (rare).

Treatment

Drug, supportive therapy

Preventive Measures

No effective prevention; early diagnosis/treatment.
