Osseous hemangioma is a hamartomatous benign tumor; any site, rare in bone. Spinal hemangioma = 3-8.1% of primary spinal tumors; low detection (asymptomatic early; late lytic lesions misdiagnosed as metastasis). Favors spine/skull; thoracic/lumbar > cervical. Age ~40; female>male.
Stiff spine; diffuse angiokeratoma; spinal-cord compression; cervical fracture/dislocation; paraplegia
(1) Etiology: unknown; tumor-like malformation/hamartoma. Vascular tissue between bone trabeculae; cavernous (spine/skull) vs capillary (flat bones/metaphyses). (2) Mechanism: tumor-like capillary/cavernous/venous sinuses between trabeculae; some true benign tumors. Gross: within bone shell, osteopenia, honeycomb, coarse sclerotic vertical ridges, dark-red vascular tissue, expansion. Microscopy: variable blood sinuses/tortuous capillaries, thin-walled dilated vessels, RBC-filled, residual trabeculae, fatty marrow.
Supportive, rehabilitation, radiotherapy
Radiosensitive; radiotherapy first; surgery for non-critical sites (distal ulna/fibula/rib); good prognosis.
It is recommended to visit the Surgical Oncology department; an appointment can be booked on this platform before the visit.
