Porphyrias are inherited/enzyme defects of heme biosynthesis, grouped as acute (neurovisceral) and cutaneous. Acute porphyrias (acute intermittent, variegate, hereditary coproporphyria) present with severe abdominal pain, neuropathy, neuropsychiatric disturbance, and dark urine, often triggered by drugs, alcohol, fasting. Cutaneous porphyrias (porphyria cutanea tarda, erythropoietic protoporphyria) cause photosensitivity and skin blistering. Treatment: avoid triggers, carbohydrate loading/hemin for acute attacks, beta-carotene/phlebotomy for cutaneous forms.
Acute: severe abdominal pain; neuropathy; neuropsychiatric symptoms; dark urine. Cutaneous: photosensitivity; blistering; skin fragility
Heme-pathway enzyme defects cause accumulation of porphyrin precursors (ALA/PBG) causing neurovisceral toxicity, or porphyrins causing photosensitizing skin damage.
Acute: hemin, glucose/carbohydrate loading, avoid triggers; cutaneous: beta-carotene, phlebotomy, sun protection
Avoid precipitant drugs/alcohol/fasting; sun protection; genetic counseling.
It is recommended to visit the Hematology department; an appointment can be booked on this platform before the visit.
