Pinealoma

Pineal gland is above diencephalon, secretes melatonin (opposite MSH), neuroendocrine transducer/biological clock, may inhibit gonads. Pinealoma (Pellizzi 1972) = precocious puberty syndrome/hypertrophia hemihypergenitalis. Causes: gonadotropin/sex steroid excess. Common tumors: pineoblastoma, pineocytoma, glioblastoma, teratoma, germinoma, seminoma, astrocytoma, ectopic pinealoma. <1% of intracranial tumors; boys.

Common Symptoms

Oculomotor palsy; somnolence; dementia; ataxia; lethargy; calcification; sluggish light reflex

Causes

Boys: exogenous androgens/feminizing adrenal tumors/estrogens. Girls: estrogen contraceptives/androgens. Familial hypertestosteronemia, adrenal hyperplasia/tumor, Leydig cell tumor, ectopic hCG-secreting tumors (CNS, embryoma, hepatoma, hepatoblastoma, teratoma, choriocarcinoma). Feminizing adrenal tumor, autonomous ovarian cysts, McCune-Albright. Excess tonics.

Treatment

Drug therapy; supportive care

Prevention

Quit smoking/alcohol; avoid spicy/overheated/cold/spoiled foods; stress management; exercise; regular lifestyle; alkaline diet.
