Thyroiditis

Thyroiditis is a group of inflammatory thyroid diseases. Classification: acute suppurative (bacterial abscess), subacute de Quervain (post-viral, painful), chronic autoimmune Hashimoto (hypothyroid), painless/silent and postpartum thyroiditis (transient thyrotoxic/hypothyroid phases), drug-induced, and Riedel fibrous thyroiditis. Clinical course varies; management depends on type and phase.

Common Symptoms

Neck pain/tenderness; goiter; thyrotoxic or hypothyroid symptoms; fever; fatigue; depends on subtype

Causes and Pathogenesis

Bacterial infection, post-viral granulomatous inflammation, autoimmune lymphocytic destruction, drug-induced (interferon, amiodarone), or fibrous infiltration.

Treatment

Antibiotics for acute; NSAIDs/steroids for subacute; L-thyroxine for hypothyroid; beta-blockers for thyrotoxic; treat underlying cause

Preventive Measures

Recognize subtype; monitor thyroid function; avoid offending drugs when possible.
