ARDS is acute diffuse lung injury causing hypoxemic respiratory failure with bilateral infiltrates not fully explained by heart failure/fluid overload, due to sepsis/pneumonia/aspiration/trauma. Treatment: treat underlying cause; lung-protective ventilation (VT 6 mL/kg PBW, plateau <30); PEEP titration; FiO2/SpO2 target; conservative fluid strategy; prone positioning in severe; neuromuscular blockers in selected severe; inhaled vasodilators as rescue; ECMO in refractory; monitor complications (VAP/barotrauma/PE); nutrition; sedation minimization.

Direct/indirect lung injury causes diffuse alveolar damage, capillary leak, and noncardiogenic edema.
Lung-protective ventilation; treat cause; PEEP; prone/neuromuscular blockers; ECMO rescue; conservative fluid
Treat sepsis/pneumonia early; lung-protective ventilation; aspiration prevention.
