The pathology of disease

Herpangina is an acute febrile enterovirus disease of infants/toddlers, summer-autumn. Caused by Coxsackie group A. Acute high fever, sore throat, drooling, anorexia, vomiting; pharyngeal injection; 2-4 mm vesicles on anterior faucial pillars/uvula/soft palate with erythematous halo, ulcerating.

Common Symptoms

Lymphadenopathy; food refusal; scattered gray-white pharyngeal lesions; myalgia; fatigue; diffuse pharyngeal mucosal injection; convulsion

Causes and Pathogenesis

(1) Etiology: Coxsackie A (1-6/8/10/22), herpes, EBV; triggers: fatigue/atopy/cold/weather change. Pediatric URT defense deficits: no nasal hairs, delicate mucosa, insufficient immune secretions, weak ciliary clearance. (2) Pathogenesis: virus enters via nasopharynx/oral mucosa, replicates in epithelial/pharyngeal/GI lymphoid tissue, spreads via lymph/blood.

Treatment

Oral care, antiviral, symptomatic/supportive therapy

Preventive Measures

Child summer disease; hygiene; avoid noisy public places. Nutrition: lukewarm light non-greasy diet; IV nutrition if needed. Topical analgesics/antivirals; boil utensils; avoid spicy/hot/acidic foods. Mung bean/bitter melon/Job's tears/porridge.
