Coxsackievirus Infection

Coxsackievirus infection is an enteroviral disease with diverse manifestations: aseptic meningitis/encephalitis/myocarditis/pericarditis/epidemic pleurodynia/herpangina; some polio-like. Group A/B; summer-autumn; GI/respiratory/transplacental transmission. Targets CNS/heart/liver/pancreas/lung/reproductive/skeletal/mucocutaneous.

Common Symptoms

Macule; papulomacular rash; dull pain; tinnitus; sepsis; convulsion; sudden death; nausea; hypotension

Causes and Pathogenesis

Picornaviridae/Enterovirus; spherical 22-30 nm; ssRNA 6000-8500 bp; 32-capsomere icosahedral capsid VP1-4 + Vpg. Group A (24 original serotypes, 23 remain; type 23 = Echo 9) and B (6 serotypes, group antigen). B causes CPE in culture; A doesn't.

Treatment

Antiviral, symptomatic, supportive therapy

Preventive Measures

No vaccine (too many serotypes). Isolate patients 2 weeks; daycare/maternity wards focus; food/hygiene; boil water; masks; feces treated with 20% lime; IVIG 3-6 mL for exposed infants; OPV may interfere; B-group vaccine in high-risk groups.
