Coccidioidomycosis is pulmonary/systemic fungal disease by Coccidioides immitis; lung = portal and most affected organ. Primary = acute self-limited respiratory; progressive = chronic often fatal systemic.
Fungemia; lymphadenopathy; respiratory failure; low fever
Dimorphic: arthrospore in nature, spherule in host; resistant to dryness/UV, sensitive to formaldehyde; 60°C/1 h kills. Inhaled arthrospores replicate in alveoli → neutrophilic inflammation → chronic granuloma. Cell-mediated immunity; racial/ABO predisposition (Black/Native American; B/AB). Pathology: pyogenic + granulomatous alternating.
Drug, supportive, surgical therapy
Avoid endemic areas.
