The pathology of disease

Malignant hypertension is a life-threatening severe BP elevation (usually >180/120) with bilateral retinal hemorrhages/exudates and/or papilledema, plus renal failure/encephalopathy. It requires urgent controlled BP reduction. Treatment: hospitalize/ICU; controlled IV BP lowering (nitroprusside/labetalol) over hours (avoid precipitous drop); investigate/treat secondary causes; manage encephalopathy/renal failure; follow-up.

Common Symptoms

Causes and Pathogenesis

Severe BP elevation causes endothelial injury, fibrinoid necrosis, and end-organ damage.

Treatment

Hospitalize/ICU; controlled IV BP lowering; treat secondary cause; manage end-organ damage

Preventive Measures

Control hypertension; screen for secondary causes.
