Hypertensive Heart Disease

Hypertensive heart disease is the cardiac consequence of chronic hypertension, including left-ventricular hypertrophy, diastolic/systolic heart failure, coronary artery disease, arrhythmias, and sudden death. Treatment: control blood pressure (ACEi/ARB, beta-blocker, CCB, diuretic); manage heart failure (GDMT); treat coronary disease/arrhythmia; lifestyle modification; long-term follow-up of cardiac function.

Common Symptoms

Dyspnea/fatigue; chest pain/palpitations; LVH on ECG/echo; history of hypertension

Causes and Pathogenesis

Chronic pressure overload causes LVH, diastolic dysfunction, and ischemia.

Treatment

Control BP (ACEi/ARB/BB); GDMT for HF; treat CAD/arrhythmia; lifestyle

Preventive Measures

Control BP early; screen cardiac function.
