Ischemic Cardiomyopathy

Ischemic cardiomyopathy is left-ventricular systolic dysfunction/heart failure due to chronic coronary artery disease and prior myocardial infarction, with ventricular remodeling, dilatation, and reduced EF. Treatment: guideline-directed medical therapy for HFrEF (ACEi/ARB/ARNI, beta-blocker, MRA, SGLT2 inhibitor); anti-ischemic therapy; assess for revascularization (CABG/PCI); device therapy (ICD/CRT) for sudden-death prevention; cardiac rehab; advanced therapy (LVAD/transplant) for end-stage.

Heart-failure symptoms (dyspnea/edema); history of MI/CHD; reduced EF/ventricular remodeling

Causes and Pathogenesis

Chronic ischemia/infarction causes myocardial scarring and adverse LV remodeling, leading to systolic HF.

GDMT for HFrEF; revascularization; ICD/CRT; cardiac rehab; advanced therapy if end-stage

Preventive Measures

Secondary prevention after MI; early revascularization; GDMT adherence.
