Acute pericarditis is inflammation of the pericardium, most often idiopathic/viral, presenting with sharp pleuritic chest pain (worse supine, better sitting forward), a pericardial friction rub, and diffuse ST elevation. Treatment: high-dose NSAIDs plus colchicine to reduce recurrence; treat the underlying cause (infection/uremia/MI/trauma); monitor for pericardial effusion/tamponade; avoid anticoagulants.
