Subacute infective endocarditis is indolent infection (usually viridans streptococci) on previously abnormal valves (rheumatic/congenital/prosthetic), presenting over weeks with low-grade fever, weight loss, fatigue, anemia, and embolic/immunologic phenomena (Osler nodes, Roth spots). Treatment: blood cultures; prolonged IV bactericidal antibiotics (penicillin/ceftriaxone plus gentamicin); surgery for heart failure, persistent infection, large vegetations, or prosthetic involvement; long-term follow-up.
Low-grade fever; weight loss/fatigue; abnormal valves; embolic/immunologic signs; positive blood cultures
Viridans streptococci (dental source) indolently infect abnormal valves, causing vegetations/emboli.
Blood cultures; prolonged IV antibiotics; surgery for HF/persistent infection/large vegetations; follow-up
Dental prophylaxis for high-risk valves; good oral hygiene.
It is recommended to visit the Cardiology/Infectious-disease department; an appointment can be booked on this platform before the visit.
