Endocarditis

Endocarditis is infection or inflammation of the endocardial surface, usually a heart valve, presenting with fever, a new/changed murmur, embolic phenomena (Osler nodes, Janeway lesions, Roth spots), and positive blood cultures. It may be infective (bacterial/fungal) or nonbacterial thrombotic. Treatment: blood cultures; prolonged IV bactericidal antibiotics; surgery for heart failure, uncontrolled infection, large vegetations, or prosthetic valves; endocarditis prophylaxis for high-risk patients.

Causes and Pathogenesis

Microorganisms seed endocardium/valves, forming vegetations and causing destruction/emboli.

Treatment

Cultures; prolonged IV antibiotics; surgery for HF/uncontrolled infection/large vegetations; prophylaxis

Preventive Measures

Dental prophylaxis in high-risk valves; treat infection.
