Preexcitation syndrome (Wolff-Parkinson-White) is an accessory atrioventricular pathway that bypasses the AV node, causing early ventricular activation (delta wave, short PR). It may cause AVRT (SVT) and, in AF, rapid ventricular response (risk of VF). Treatment: asymptomatic - risk stratification; symptomatic - catheter ablation of accessory pathway (curative); acute SVT - AV-nodal blockers; in AF with WPW - avoid AV-nodal blockers, use procainamide/cardioversion.
Palpitations/SVT; delta wave/short PR on ECG; risk of rapid AF/VF
Congenital accessory AV pathway bypasses the AV node, causing preexcitation and reentrant tachycardia.
Ablation for symptomatic; acute SVT - AV blockers; AF with WPW - avoid AV blockers, procainamide/cardioversion
Risk stratify; ablation is curative; avoid AV blockers in preexcited AF.
It is recommended to visit the Cardiology/EP department; an appointment can be booked on this platform before the visit.
