Atrial Fibrillation

Atrial fibrillation is a supraventricular tachyarrhythmia with disorganized atrial activation and irregularly irregular ventricular response, classified as paroxysmal, persistent, long-standing persistent, or permanent. Clinical: palpitations, dyspnea, embolic stroke risk. Treatment: rate control (beta-blocker/CCB/digoxin); anticoagulation based on CHA2DS2-VASc score (DOAC/warfarin); rhythm control (cardioversion/antiarrhythmics) for selected; catheter ablation for symptomatic refractory; treat underlying causes (HTN/thyroid/OSA/valve disease).

Causes and Pathogenesis

Multiple reentrant atrial wavelets cause disorganized atrial activation and irregular ventricular response.

Treatment

Rate control; anticoagulation (CHA2DS2-VASc); rhythm control/ablation for symptomatic; treat causes

Preventive Measures

Control HTN/thyroid/OSA; anticoagulate for stroke prevention; avoid triggers.
