Heart failure is a clinical syndrome of ventricular dysfunction causing pulmonary/systemic congestion and reduced exercise tolerance, classified as HFrEF (EF<40%), HFmrEF (40-49%), or HFpEF (>=50%). Clinical: dyspnea, orthopnea, edema, fatigue. Treatment: GDMT (ACEi/ARNI, beta-blocker, MRA, SGLT2i for HFrEF); diuretics for congestion; treat underlying cause (ischemia/HTN/valve/diabetes); device therapy (CRT/ICD); cardiac rehab; transplant/LVAD for end-stage.
