Acute heart failure is sudden-onset severe ventricular dysfunction presenting as acute pulmonary edema or cardiogenic shock. It is an emergency. Clinical: severe dyspnea, orthopnea, pink frothy sputum, hypoxemia, hypotension. Treatment: hospitalize/ICU; oxygen/noninvasive ventilation; IV loop diuretics; vasodilators (if BP permits); inotropes/vasopressors for cardiogenic shock; treat precipitant (ischemia/arrhythmia/valve); mechanical circulatory support if needed.
Severe dyspnea; pulmonary edema; hypoxemia; hypotension/cardiogenic shock; acute onset
Acute precipitant (ischemia/arrhythmia/valve/drug) causes sudden ventricular decompensation.
Oxygen/NIV; IV diuretics; vasodilators; inotropes/vasopressors; treat precipitant; MCS if needed
GDMT adherence; early treatment of precipitants.
It is recommended to visit the Cardiology/Emergency-ICU department; an appointment can be booked on this platform before the visit.
