Papillary muscle dysfunction is malfunction of a mitral-valve papillary muscle, most often due to ischemia or myocardial infarction, causing mitral regurgitation. Clinical: murmur, dyspnea, heart failure. Treatment: treat underlying ischemia (revascularization); heart-failure therapy; mitral-valve surgery (repair/replacement) for severe symptomatic regurgitation; differentiate from papillary muscle rupture (emergency).
Dyspnea/heart failure; mitral regurgitation murmur; history of ischemia/MI
Ischemia/infarction impairs papillary muscle function, causing mitral regurgitation.
Treat ischemia/revascularization; GDMT for HF; surgery for severe regurgitation
Secondary prevention after MI; treat ischemia.
