Fungal Pericarditis

Histoplasmosis/coccidioidomycosis/actinomycosis/nocardiosis can involve pericardium. Rare; suspect in lymphomas/leukemia/immunosuppressed with pericarditis signs.

Common Symptoms

Pericardial inflammation; pleural effusion; lymphadenopathy

Causes and Pathogenesis

Histoplasma most common; US; inhalation of bird/bat droppings; urban digging/blasting. Other: Aspergillus/yeast/Candida; risk factors: IV drug use, immunosuppression, broad antibiotics, post-cardiac surgery. Coccidioidal: inhaled soil/dust spores. Pathogenesis: histoplasmal pericardial fluid = rapid large serous/sanguineous exudate, high protein, PMNs; other fungi slower; occasional organization/constriction/calcification.

Treatment

Drug therapy, supportive therapy

Preventive Measures

Avoid travel/residence in endemic areas; protective gear around dust/bird/bat droppings; treat primary disease.
