Sporadic encephalitis is an acute encephalitic illness presenting with fever, headache, altered consciousness, focal neurologic signs, and seizures, occurring sporadically (not in an epidemic). Causes: herpes simplex virus, other viruses, autoimmune, and unknown. It is a medical emergency. Diagnosis: CSF, EEG, MRI. Treatment: empiric acyclovir pending HSV exclusion, supportive ICU care, and treat identified cause (autoimmune encephalitis requires immunotherapy).
Fever; headache; altered consciousness; focal neurologic signs; seizures; sporadic (non-epidemic) onset
Various viruses (HSV etc.), autoimmune, or unknown cause inflame brain parenchyma, producing encephalitic symptoms.
Empiric acyclovir pending HSV exclusion; ICU supportive care; immunotherapy for autoimmune encephalitis
HSV vaccination where available; early recognition of encephalitis.
It is recommended to visit the Neurology department; an appointment can be booked on this platform before the visit.
