Brazilian purpuric fever (BPF) is an acute fulminant pediatric disease first recognized in São Paulo, Brazil in 1984, presenting with high fever, abdominal pain, vomiting, purpuric rash, and shock with rapid death. Over 90% had purulent conjunctivitis ~2 weeks before. A 1985 Pan American/CDC task force defined it as a new disease (BPF); in 1986, H. influenzae biogroup aegyptius (HIBA) was isolated from blood/CSF/purpura of typical patients.
Shock; hypotension; diarrhea; abdominal pain; GI bleeding; confusion; cyanosis; oliguria
(1) Etiology: Originally Koch-Weeks bacillus, named H. aegyptius by Pittman (1950), then H. influenzae biogroup aegyptius (HIBA) after Killa (1976) noted its phenotype resembled H. influenzae biotype IV. BPF-clone HIBA from typical patients carries a 0.024 MDa plasmid (control conjunctivitis strains have 0.002 MDa or none); whole-cell electrophoresis, 16+23S RNA probing, and extracellular protein studies distinguish BPF clones from controls. (2) Pathogenesis: HIBA invades eye/nose/throat and multiplies locally; invasive BPF clones cause bacteremia and release endotoxin (mean 675 µg/L in patients vs 25 µg/L in healthy children)—endotoxin likely causes multiorgan damage. Autopsy shows widespread petechiae/purpura, microthrombi in small vessels, hemorrhage/necrosis, cerebral edema without inflammation, pulmonary congestion/edema/hemorrhage, adrenal hemorrhage, marked lymphoid depletion, and ischemic necrosis of distal extremities/ears/nose—no vasculitis.
Drug therapy, rehabilitation
Curing conjunctivitis does not prevent BPF; some recommend topical ampicillin/chloramphenicol plus several days of systemic treatment. Surveillance in endemic areas tracks BPF clones and predicts outbreaks.
It is recommended to visit the Infectious Diseases department; an appointment can be booked on this platform before the visit.
