Kala-azar is chronic endemic L. donovani infection, formerly north of Yangtze River. Source: patients/infected dogs; sandfly vector May-August. Amastigotes in patient blood develop into flagellates in sandfly; 7 days later re-injects. Parasite lives in blood/liver/spleen/bone marrow/nodes.
Splenomegaly; papule; eosinophilia
Zoonotic; human-animal/animal-animal transmission. Source: patients/infected dogs; sandfly vector. May-August sandfly season.
Drug, surgical, supportive therapy
Treat patients; cull infected dogs; spray DDVP/trichlorfon. Pre-season mass screen/treat; 1-year follow-up; dog registration/culling in hill endemic areas; indoor latrine spraying May-September; ventilation/dry floors; repellents; fine-mesh bed nets.
