Diabetic Gastroparesis

消化内科

Common symptoms

腹胀、早饱、腹痛、烧心、恶心、食管反流症状

The pathology of disease

Diabetic gastroparesis is delayed gastric emptying without mechanical obstruction, caused by autonomic neuropathy and interstitial-cell-of-Cajal injury in long-standing diabetes. Clinical: postprandial fullness, bloating, nausea, vomiting of undigested food, early satiety, weight loss, and erratic glycemic control (unpredictable carbohydrate absorption). It worsens with hyperglycemia; diagnosis by gastric-emptying scintigraphy.

Common Symptoms

Postprandial bloating; nausea; vomiting of undigested food; early satiety; weight loss; erratic blood glucose

Causes and Pathogenesis

Chronic hyperglycemia damages vagal autonomic nerves and gastric interstitial cells of Cajal, impairing antral motility and pyloric coordination; delayed emptying.

Treatment

Optimize glycemic control; low-fat low-fiber small meals; prokinetics (metoclopramide, erythromycin); antiemetics; gastric electric stimulation in refractory

Preventive Measures

Tight glycemic control; small frequent low-fat meals; avoid high-fiber/fat; monitor hydration.
