Hereditary spastic paraplegia (familial spastic paraparesis) is a group of inherited neurodegenerative disorders characterized by progressive spastic weakness and gait disturbance of the legs, due to degeneration of the corticospinal tracts. It may be pure (leg spasticity alone) or complicated (with ataxia, neuropathy, cognitive impairment). Most are autosomal dominant. Onset varies. There is no cure; treatment is supportive with physical therapy, antispasticity agents (baclofen), and walking aids.
Progressive spastic leg weakness; stiff gait; urinary urgency; autosomal-dominant family history; variable age onset
Inherited mutations cause length-dependent corticospinal-tract axonal degeneration, producing progressive spastic paraparesis.
Supportive; physical therapy; antispasticity agents (baclofen); walking aids; treat urinary symptoms
Genetic counseling; family screening; supportive rehab.
It is recommended to visit the Neurology department; an appointment can be booked on this platform before the visit.
