The pathology of disease

Hereditary ataxia is a group of inherited, progressive ataxias due to degeneration of the cerebellum and/or its connections/spinal cord. Main entities: Friedreich ataxia (autosomal recessive, with cardiomyopathy/scoliosis), spinocerebellar ataxias (autosomal dominant, many subtypes), ataxia-telangiectasia, and others. Clinical: progressive gait/limb ataxia, dysarthria, nystagmus, and associated systemic features. There is no cure; treatment is supportive, physical/occupational/speech therapy, and genetic counseling.

Common Symptoms

Progressive gait/limb ataxia; dysarthria; nystagmus; inherited family pattern; associated cardiomyopathy/scoliosis/immune features by subtype

Causes and Pathogenesis

Inherited genetic mutations cause progressive degeneration of cerebellar and spinal pathways, producing ataxia.

Treatment

Supportive; physical/occupational/speech therapy; treat associated features (cardiomyopathy); genetic counseling

Preventive Measures

Genetic counseling; family screening; supportive rehab.
