Vascular parkinsonism (arteriosclerotic parkinsonism) is parkinsonism due to cerebral subcortical small-vessel disease, typically presenting as lower-body predominant gait difficulty, magnetic gait/freezing, and falls, with little tremor and poor levodopa response. It is more common in elderly patients with vascular risk factors. Treatment: control vascular risk factors (BP/diabetes/lipids), gait rehabilitation, and physical therapy; levodopa response is limited.
