神经内科

Common symptoms

眼睑痉挛、虎眼征、智能减退、痴呆、共济失调、吞咽困难、静止性震颤、腱反射亢进、口齿不清

The pathology of disease

Hallervorden-Spatz disease (now pantothenate kinase-associated neurodegeneration, NBIA/PKAN) is an inherited neurodegeneration with brain iron accumulation, presenting in childhood with progressive dystonia, rigidity, parkinsonism, dysarthria, dementia, and pigmentary retinal degeneration. MRI shows the 'eye-of-the-tiger' sign in the globus pallidus. There is no cure; treatment is supportive (antidystonic agents, rehab, nutrition/respiratory support).

Common Symptoms

Childhood progressive dystonia/rigidity; dysarthria; dementia; retinal degeneration; MRI 'eye-of-the-tiger' sign

Causes and Pathogenesis

PANK2 mutation causes abnormal iron accumulation in the globus pallidus, producing progressive extrapyramidal/cognitive degeneration.

Treatment

Supportive; antidystonic agents; rehab; nutrition/respiratory support; genetic counseling

Preventive Measures

Genetic counseling; supportive care.
