Central pontine myelinolysis (osmotic demyelination syndrome) is demyelination in the central pons classically caused by overly rapid correction of chronic hyponatremia. Clinical: acute pseudobulbar palsy, flaccid then spastic quadriparesis, and altered consciousness ('locked-in' state in severe). MRI shows central pontine lesion. Prevention is key: correct hyponatremia slowly. Treatment: supportive care, slow sodium normalization, and rehab; recovery may be partial.
Pseudobulbar palsy; flaccid then spastic quadriparesis; altered consciousness; recent rapid sodium correction
Rapid correction of chronic hyponatremia causes osmotic demyelination of central pontine oligodendrocytes.
Supportive; slow sodium correction; rehab; re-lowering sodium in selected cases
Correct hyponatremia slowly; monitor electrolytes.
It is recommended to visit the Neurology department; an appointment can be booked on this platform before the visit.
